Any idea what this is from or what it is?? It feels hot slight itch on neck but it’s not a itchy rash if that makes sense it’s more hot (I’m not in a hot environment I’m cold right now)

What are those spots? I got same thing on other leg and it's sometimes itchy, I think area is getting bigger over years. I have been to a dermatologist, she told me to use Keladerm ointment and take Calcium Dobesilate, I did it for 5 months and nothing changed, it's maybe even worse.
