I just loaded the game and... Geralt and everyone around were broken. The body came back later, but everyone was standing there with their heads torn off, and their hats and objects, if they were holding them, were floating around in the air. Then game crashed.

When I started the isle of mists quest it said that I failed this side quest but I completed it before starting the main quest

Doing a NG+ on Death March with the deflection enchantment on my Ursine armor. No one shot deaths from bandit arrows. Highly recommended.
