In studies that track the incidence of certain events in a given population. How do researchers differentiate between an actual increase in the event as opposed to people just reporting/diagnosing that event more often/correctly? Is it even possible to separate the two?

What is the current state of evidence for/against the existence of dark matter? Is it still the overwhelmingly strongest theory to explain current observations of the universe? How do supporters reconcile observations where it has come up short?
