We often read of battles in WWI which resulted in the deaths of tens-of-thousands of men on both sides, if not hundreds of thousands. Why were losses so massive? Did commanders not make an effort to change tactics rather than just repeat the attack in the same fashion?

Why was conventional conflict/warfare phased out? Why do countries/governments go to war against certain individuals or groups within a nation to defeat it, rather than declaring that they are actually attacking the nation itself.
