American politicians are referred to by their official titles (eg President Bush, Secretary Clinton, Mayor Buttigeg) long after they have left office. When did this start to be commonplace? Are there reasons why this is common in the US but not elsewhere in the world.

The Allied campaign in North Africa in 1941 was called “Operation Crusader”. This seems a wilfully insensitive name for an operation in Muslim lands. Was any thought given to this at the time? When did “crusading” take on negative connotations?
