If the blood-brain barrier is able to stop most pathogens from entering the brain, is there any reason why this barrier isn't used elsewhere in our bodies to essentially 'scrub' out pathogens from our blood?

When people take a blow to the head they're told not to fall asleep otherwise they can lapse into a coma. Why does stopping them from falling asleep stop this from happening?

Oxygen is a 'free radical' that can cause cell damage via ionisation, increasing risk of types of cancer and other medical conditions. Were rates of these conditions were less/none pre-oxygen-catastrophe? And were they high when the atmospheric oxygen percentage was higher than it is now?
