PLEASE HELP bumps / sheeths around hair follicles on chin. Had for the longest time and visited many derms with no relief. Any ideas please?

Please help- had this for 4 years. White bases around hair follicles on chin, and some hairs come out thick, silvery / white. Derms have not helped.
