Did gunners on WWII bombers, specifically guys in the top turrets, ever accidentally shoot the plane they were flying in? Also, when the bullets fired in aerial battles eventually landed, did they ever kill anyone on the ground?

In “Master and Commander” the commander of the ship’s Marines accidentally shoots the ship doctor while trying to hit a bird. Would he have gotten in trouble for this? What rules were there about firearms on board Napoleonic ships?

In the series “The Pacific” John Basilone wins the MoH in 1942 but is killed on Iwo Jima in 1945 after volunteering to return to combat. Did this organically happen to any Americans in the Pacific Theater? How did troop rotations work? Was it possible to be in battles three or four years apart?

In “Master & Commander” we see marines in the rigging firing at enemy crew on the other ship’s deck - would these marines ever just fire at their counterparts in the enemy’s rigging?
