Are thoughts always in our mother tongue? How is a person's thoughts processed if they don't know a language?

When people think, they think in their mother tongue. Is this some sort of illusion or way our brains allow us to comprehend our thoughts? What if someone never learned a language. What exactly would they hear in their head when they have thoughts?

In movies and TV shows, if someone gets shot in the head, we often hear the victim groan in pain. Is this realistic? Would the person not die before they would even be able to register the pain then express it audibly?
