In all 50 United States, why do all of them use the presidential system of government instead of a semi-presidential or parliamentary system, and all but one are bicameral?

Why do almost all countries with parliamentary systems of government all have a ceremonial head of state, rather than just having the head of government also be the head of state?

The US Constitution's seventh amendment gives people the right to a jury trial in civil cases where the amount is over $20. Has anyone ever tried exercising this right over extremely petty amounts?
