In some accounts of WW1 there is mention made of front-line soldiers thinking it would go on "for years and years," and some thought this as late as the end of 1918. Could someone better explain this viewpoint? Was it a genuine surprise when the stalemate was broken in the autumn of 1918?

Recently re-watched The Pacific and noticed one of the major themes was psychological trauma experienced by soldiers. My question is, in the PTO during WWII, how "bad" would a soldier have to get before they pulled him off the line? Was there a system in place or was it subjective to the CO?
